About My Health Phone

What is My Health Phone?
My Health Phone is a service offered by Proximiti Communications combining a touch-screen Android cell phone with Proximiti Health Network applications providing services designed to enhance a senior’s ability to live independently.   Proximiti’s software is adapted to the Samsung Galaxy Mini phone to optimize the customer experience.

  • Large-type single-button access for contacting "teammates," such as friends, family, or caregivers. The Senior or a designated teammate uses a web interface to customize teammates.
  • Fully customizable regular check-in and reminder calls
  • Capability to send alerts for missed reminders or check-ins, or negative responses to specified teammates. The Senior or teammate configures these using a web interface.
  • My Internet Lockbox. This service provides remote storage and secure access to essential documents such as Living Wills, Durable Healthcare Power of Attorney and HIPAA release forms.

My Health Phone offers so much more than a traditional personal emergency alert system (sometimes called a PERS device).   PERS devices such as pendants or fobs communicate over a walkie-talkie-like radio with a base station in the home.   The Base station connects to regular telephone service.   This technology is much more limited than My Health Phone because they can only be used for emergencies and typically do not work outside the home.

I live in a rural area..  Will My Health Phone work for me?

The Proximiti My Health Phone service sounds perfect for my parent.  Can I order the service for them?
You can set up and maintain Proximiti Health services online by visiting myhealthphone.com.   The senior and / or an authorized caregiver such as a family member, friend or home care agency can configure the service—it’s very flexible!   When you set up My Health Phone service, you must accept Proximiti’s Terms of Service and agree that teammates may be able to access certain My Health Phone account information.  

If an individual subscribes to My Health Phone service on behalf of another person, services are conditional upon the end-user’s understanding and acceptance of these conditions.   Subscription by a third party on behalf of an end-user without such consent and acceptance is unlawful.   Similarly, credit card information entered on behalf of another party and acceptance of charges is only allowed only where the card holder understands and fully consents and authorized the Service charges.  
Do I have to purchase the Galaxy Mini phone in order to get My Health Phone Service?
Today, My Health Phone service requires purchase of the Samsung Galaxy Mini from Proximiti.   We are currently working on enhancing the My Health Phone software to run on other Android and Blackberry smartphones.   When our "stand alone" My Health Phone application is available, we will post an update.  
What do I get with my phone?
Your phone will come in the factory box.   The box will include:  

  • Galaxy Mini phone
  • home wall charger
  • wall adapter (if necessary)
  • battery
  • "USB" computer connector
  • 2GB micro SD Card
  • SDHC Converter (for using micro SD Card in a computer or camera)
  • Proximiti SIM Card (for GSM cellular service)
  • Samsung Quick Start Guide

The battery, micro SD Card and SIM card will already be installed in the phone.   My Health Phone software will be pre-installed on the phone.   Proximiti’s technicians have charged the battery and tested the phone to ensure it is working status at the time of shipment.

Do you offer any accessories like car charges or carrying cases that fit the Galaxy Mini?
Yes.   Proximiti offers a full range of optional accessories for the Galaxy Mini at very competitive prices.   These include but are not limited to car chargers, extra home chargers, cases, extra batteries, Bluetooth headsets and larger micro SD storage cards.   These items change from time to time as new products come on the market.   For the accessories we offer, we guarantee compatibility with the Galaxy Mini.   The latest optional accessories are offered to subscribers during the sign-up process.   If a subscriber would like to order accessories at a later date, log into the Proximiti Health web portal at www.proximiti.com/health1/login and click on the Store menu option or icon.  

Account Setup

Do I have to sign a contract or is there a minimum commitment period for the service?  If I cancel the service, do I need to return the phone?
Unlike most other cell phone services, My Health Phone does not require you to sign a contract and service is provided on a month-to-month basis.   The subscriber may cancel the service at any time..   The Galaxy Mini phone associated with My Health Phone service is purchased by the subscriber at time of subscription and does not need to be returned when service is canceled..   By replacing the SIM card contained within the phone, the Galaxy Mini may be utilized with other GSM based cellular service providers as the phone is not "locked".  
How long will it take to set up My Health Phone account?
Your My Health Phone account and access to the Proximiti Health Network will be setup within minutes of completing the signup process.   You can begin the process of setting up teammate lists, check in calls, and reminder services at that time.   However, it will generally take approximately 5 to 7 business days to establish the cellular telephone service associated with your Health Phone account.  
How long will it take to get my Health Phone Galaxy Mini?
Proximiti will begin configuring the Galaxy Mini the same day you place the order.   Your new Galaxy Mini will arrive within 5 to 7 business days via UPS.  

If you are porting an existing cell phone telephone number to Proximiti, your new Galaxy Mini will arrive within the normal 5 to 7 business day interval.   However, in order to ensure that you do not have a break in cellular service, we will not begin the process of moving your number to your new phone until after we have confirmed you have received it.   Once we have confirmed delivery, we will initiate moving your number, which should take less than 2 days.  
How will I set up my reminder services?  How do I specify my teammates?
During the set up process, each subscriber will set up a User ID for their account and will be asked to establish a password.   Once the My Health Phone sign up process is complete, the subscriber can access their account at:  www.proximiti.com/health1/login   The subscriber will be prompted to enter their User ID and Password.  

Once signed in, the subscriber may utilize the available menus to establish and/or update various aspects of their My Health Phone service such as wellness calls, reminder types, reminder times, message delivery preference (voice call, SMS text message or email) and their list of teammates.  
Can I change other phone features?
Yes.   Many features of the phone such as the ring tone, ring loudness, and display brightness are controlled by the phone’s operating system.   Changes to these phone features can be made through the main Settings menu of the phone.   To learn more about what features you can control and how to make changes, download the Galaxy Mini full user manual at this web site:   http://downloadcenter.samsung.com  
What is the Emergency Medical Card and My Internet Lockbox service?
My Internet Lockbox is designed to allow individuals to store critical medical information and allow for access in medical emergencies.  

Proximiti’s www.myinternetlockbox.com is an online web application that allows a person or family to set up and store a wide range of medical and other information.   My Internet Lockbox can store emergency contacts, copies of important medical forms, and listings of prescriptions and allergies, and much more.   The subscriber can permit access to any of their information by medical personnel via web interface or fax.   The access information is printed on an emergency card, which can be easily stored in a wallet or purse.  

Emergency medical personnel can either go to www.911cardinfo.com or dial one of our domestic 800 numbers or International phone numbers and enter the unique ID of the card to retrieve the relevant medical information almost instantly via fax or online.  

Families can also use Proximiti’s My Internet Lockbox as an online safety deposit box for storage of other important documents, insurance carrier information, professional advisor contact information, and even pictures of home furnishings.   For instant access to this information a second level of security is required (a PIN).  
Within your web site, you mention "opening" the Android phone.  What do you mean?
When an android phone is turned open it opens to a page similar to the image below.   To open the Android phone, you place your finger on the screen over the green unlock control (left bottom) and then slide your finger to the right across the phone’s screen.   This sliding/swiping motion opens the phone and allows the My Health Phone software to load as your home page.   The purpose of this initial screen is to make sure that inadvertent screen-touches while in your pocket, purse or hand do not result in unwanted calls or text messages.   You may know this as "pocket dialing."  

Please note:  the screen of your phone is made to use your finger to touch, tap and slide across its surface.   Use of your finger on the screen will not harm the phone or screen.  

Customer Service and Troubleshooting

Can you describe Proximiti’s Customer Service process and procedures?
Proximiti provides telephone assistance to subscribers for routine service matters.   We also offer live chat on our web site during business hours.   We recommend that you contact our customer service department for all inquiries.   Customer support lines are open seven days a week, 24 hours a day.   Where our service technicians may not be immediately available, customers may be asked to leave a message giving their My Health Phone telephone number, summarizing the problem and providing an alternative contact number.   Caregivers and teammates may initiate service calls on behalf of the subscriber where they have the subscriber’s permission.   Approximately 99% of all questions received by Proximiti can usually be handled while on the phone.   For the 1% of questions or issues that are more complicated, we may need to investigate further.   In this case our customer care department will follow-up with the appropriate party in a timely manner.  
What if I forget my User ID and/or Password?
Subscribers who forget their User ID or Password should access the following web site:   www.proximiti.com/health1/login and click the link labeled "Forgot your ID or password?" and follow the directions found there.   If you reset your password on the web, remember to change the password using your My Health Phone settings.  
My Proximiti Health Phone won’t work.  What is the likely problem?
If your phone display is not blank, but appears to be frozen, we suggest pressing and holding the top button on the right hand side of the phone (the on/off switch) for 3 seconds.   The phone will power off.   Wait 30 seconds and press the same button again for 3 seconds to power the phone back on.  

If your phone display is blank, press and hold the top button on the right side for 3 seconds to power the phone on.   If the phone screen remains blank after 30 seconds have elapsed, the most likely problem is an uncharged battery.   Charge the phone for at least 2 hours before restarting the phone using the cord and wall charger provided with your phone.  

To help ensure that the phone is ready to use at all times:  

  • We recommend that subscribers put the charging station for the phone next to their bed and plug the phone into the charger nightly.
  • Proximiti’s embedded software will trigger an alert when the phone falls below a 10% power level as a reminder to charge up.
  • The My Health Phone service can be configured to alert teammates that calls, alerts, and queries are not being answered.

So while you can’t charge the phone remotely, key people will be alerted if the phone is not responding, which may indicate a dead battery.  
If the Proximiti provided phone quits working, what happens?
We offer a 90 day full warranty on the phone.   Problems are rare but can occur.   We typically ship a new phone configured for that user and then ask them to return the existing phone in the box that we send.   Outside the warranty period, Proximiti will work with you to get your service back up and operational.  
What is Proximiti’s privacy policy for My Health Phone and My Internet Lockbox services?
